Hampshire County Council is seeking a Going Home Worker to support reunification journeys for children in care. You will work directly with children, families and professionals to ensure voices are heard and plans aim for safe, stable outcomes.

The role focuses on building trusted relationships, coordinating services and embedding best practice across the Going Home pathway within Hampshire.

Competenze

• Experience working with children, young people and families in a support or care role.
• Strong communication skills with a wide range of people.
• Good organisation, ability to write reports and manage priorities.

Mansioni

• Build positive relationships with children, parents, carers and wider networks to support reunification planning.
• Develop and progress robust Going Home plans that enable children to return home safely.
• Collaborate with Social Workers and partner agencies to coordinate support and inform decisions.
• Monitor progress, maintain records and provide updates on reunification activity.
• Signpost families to practical support and strengthen community networks.
