Job summary

Durham County Council is recruiting three Family Hub Practitioners to join its Early Help Service. One permanent part-time position and two temporary full-time posts (to 31 March 2027 and 31 December 2027) are available.

You will assess needs and deliver interventions for families, often working with health and social care colleagues to support the Start for Life agenda. Successful candidates will have a Level 3 qualification and DBS clearance if required.

Qualifications

• Recent experience of working with and supporting vulnerable children, young people and families in home or community settings.
• Experience of group facilitation and delivering planned interventions that improve outcomes.
• DBS Enhanced Disclosure required if successful.

Responsibilities

• Assess needs of children, young people and families and deliver high-quality interventions one-to-one and in groups.
• Refer and signpost families to other services for additional support.
• Collaborate with midwifery, health visiting and other key services to deliver Healthy Babies offers.
