This role involves working as a member of the Family First Service, delivering targeted interventions to families with children aged 0-18 years with complex needs. Responsibilities include supporting families to improve outcomes like school attendance, reducing risky behaviors, and enhancing employability.
Family First Case Managers will adopt a multi-disciplinary approach, conducting holistic assessments, developing outcome-focused family plans, and coordinating multi-agency support using models like Signs of Safety and Wellbeing. They will deliver evidence-based programs such as parenting courses and consider approaches like Contextual Safeguarding.
Additional duties include developing prevention programs, participating in practice improvement, and leading thematic activities. The role requires engaging families assertively, undertaking early help assessments, acting as lead professional, coordinating service delivery, and maintaining accurate records.
Case Managers will also provide direct parenting support through various programs and in-home support, ensuring the voice of the child is central in all activities.
