We are seeking a dynamic and suitably qualified and experienced health care professional to support the strategic delivery of the Greater Manchester Child and Adolescent Lead Provider Collaborative (GM CAMHS LPC). Working as part of a wider multi-professional programme team across all providers, to ensure quality responsibilities are met, and to support the collaborative and to be self-assuring and self-regulating.
As an LD Case Manager in Greater Manchester CAMHS LPC the post holder will work as park of a dynamic team in delivering an effective service that oversees all care pathways for young people who require inpatient Mental Health Inpatient care within NHS and Independent sector providers, specializing in the cohort of young people with a Learning Disability and/or autism diagnosis
The post holder will provide case management direction for a caseload of young people and will over see the assessment, intervention and discharge planning throughout their inpatient journey. The role will include robust oversight of the access assessment, whilst ensuring young people are placed in the least restrictive environment with goal orientated treatment

• Provision of Learning Disability and Autism clinical expertise to the LPC
• Ensuring effective LDA care pathways, within and across LPC boundaries
• Resolution of complex or disrupted care pathways in conjunction with
colleagues across England (and within UK for patients placed in services
outside England)
• Support the LPC in ensuring that services are compliant to all specifications and guidance.
• Ensuring the delivery of high quality services that are value for money
• Ensuring case management systems and record keeping are maintained and kept up to date (e.g. NCMS, AT)
• Contribute to review all LDA patients within our commissioned remit in
line with Standard Operating Procedures (SOP)
• Supporting with training, awareness and resources related to CETRs
and associated processes e.g. DSRs
• Attendance at GM CETR oversight panel and associated
Transforming Care meetings, taking on a lead role including regular
reporting, highlighting quality assurance intelligence, working to tackle
problems and providing recommendations for improvement

What happens if I am offered the position after interview?
The hiring manager will make contact with you to verbally offer you the position. The hiring manager will then inform Recruitment of the decision and provide relevant paperwork. You will be sent a formal conditional offer via email.
What pre-employment checks will I need to complete?
By conducting pre-employment checks, the recruitment team will verify that you meet the pre-conditions of the role you have been offered. Pre-employment checks will be carried out according to NHS Employment Check Standards . The checks are:

What happens when pre-employment checks are complete?
Recruitment will liaise with you and the hiring manager to arrange a start date for your new position. You will then be booked on to a Trust Welcome Session and be sent your Pennine Care NHS Terms and Conditions.
