Job summary

Abbots Care is looking for caring individuals to help clients live life to the fullest at home. You will visit service users, provide personal care, monitor health, and support independence within a Care Plan.

Shifts run 7am–2pm or 4pm–10pm, with alternate weekends. Competitive hourly rates apply, with mileage paid and ongoing training opportunities.

Qualifications

  • NVQ level 2 or above in Health and Social Care.
  • Access to your own vehicle is required for the role.

Responsibilities

  • Help people leave hospital and return home independently.
  • Visit service users at home to provide care per the Care Plan.
  • Administer medication, monitor vital signs, assist with exercises and domestic tasks.
  • Liaise with medical professionals such as district nurses and pharmacies.

The rolewill include tasks such as:
  • Helping people to leave hospital as soon as is practicably possible and to safely transition back to living at home as independently as possible.
  • Visiting Service Users in their own homes to provide appropriate care and support as defined in the Care Plan.
  • All aspects of personal care, administering medication, monitoring of vital signs, exercise and domestic tasks if needed
  • Liaising with medical proffessionals such as district nurses and pharmacies.

The aim is to rehabilitate and maximise potential, to support and enable Service Users in a range of activities to promote independent living and monitor their well-being.

Working hours are between 7am-10pm with shifts patterns of7am-2pm and 4pm-10pm which areinclusive of alternate weekends.

We offer high pay rates of £13.43ph on weekdays and £15.50ph on weekends.

* the hourly rate is subject to holding a NVQ level 2 or above in Health and Social Care
