Job summary

A leading health service organization in North Wales is seeking a dedicated Support Worker to join the Enhancing Lives Team. This role involves providing support and wellbeing interventions to vulnerable individuals facing complex challenges such as substance use and mental health issues. Qualifications

  • Degree or equivalent experience in relevant field of practice.
  • Significant experience of working within relevant service.

Responsibilities

  • Participate as part of the Enhancing Lives Team providing support to vulnerable individuals.
  • Collaborate with the multi-disciplinary team to ensure quality evidence-based care.
  • Make limited assessments of individual needs and follow protocols.

Skills

Awareness of substance use

Job description

The post holder will be required to participate as part of the Enhancing Lives Team, providing support and wellbeing interventions to service users in the community. The Support Worker will play a significant role in the Enhancing Lives Team; aiming to support the outcomes of engaging/re-engaging vulnerable individuals experiencing complex needs in relation to issues including substance use, homelessness, mental health and offending behaviour, which is often underpinned by trauma.

Main duties of the job

Support Workers will undertake roles and duties delegated by senior members of the team. Working within the boundaries of the Enhancing Lives Team, the scope of the role enables the post holder to make a limited assessment of individual need and act upon their assessment.

They will work collaboratively with the multi-disciplinary team to meet the needs of people being supported by providing quality evidence based care. The post holder will follow explicit protocols, policy and procedures and provide supervision to more junior staff.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qulifications & Knowledge

Experience
  • Significant experience of working within a relevant service as a senior HCA / HCSW/support worker.
  • Experience of working at as a support worker within a relevant community setting using an assertive outreach approach
