  • Experience in complex or critical care, including tracheostomy and ventilation.

Field-based role with frequent travel across the Midlands

Permanent, Full-Time position with on-call telephone support on a rota system

Salary: £49,000-£53,000 + £3,000 car allowance and benefits

Are you passionate about delivering exceptional care and making a real difference in the community? We are looking for a Clinical Lead responsible for managing a caseload of community care packages across London and surrounding areas.

Your responsibilities will include ensuring high standards of clinical practice, managing risks to safeguard staff and service users, and building trusted relationships with internal teams, service users, and their families. Part of Acacium Group, Thornbury Community Services delivers high-quality care with a team of dedicated nurses and care staff, providing complex care at home or in the community, 24/7 as needed. Everyday you will...
  • Undertake initial holistic nursing assessments with service users and their families
  • Develop individualized care plans in collaboration with service users, families, and healthcare professionals
  • Create risk management plans, escalate high risks, and monitor their effectiveness
  • Deliver clinical training tailored to client needs for staff within packages
  • Ensure clients are regularly reviewed in line with policies and national guidelines
