Job Description:

NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK, employing around 40,000 staff in various clinical and non-clinical roles. We provide acute hospital, primary, community, and mental health services to over 1.15 million people, expanding to 2.2 million with regional and national services.

This is a fixed-term, full-time position for 9 months, with a shift pattern from Monday to Friday, 9 am to 5 pm.

We offer an exciting opportunity for Nursing, AHP, or Pharmacist professionals to join the community element of the Home First Response Team as an advanced frailty practitioner within Renfrewshire HSCP.

The role is full-time, but part-time options may be considered temporarily to enhance service resilience. This is a secondment opportunity, requiring approval from your current line manager before application.

The post is within a multidisciplinary team supporting individuals with frailty to maintain independence and wellbeing in their communities, aiming to prevent unnecessary hospital admissions.

We welcome applications from experienced practitioners interested in service improvement and self-development, particularly from the NMAHP and pharmacy workforce. This Band 7 specialist role involves working across primary and secondary care interfaces, facilitating communication among multidisciplinary teams in acute and community settings. The practitioners operate as a virtual network, enabling high-level joint clinical decision-making to ensure timely and appropriate care, thereby reducing avoidable hospital presentations and supporting timely discharge.

Given the autonomous and specialist nature of the role, we are committed to supporting successful candidates in undertaking postgraduate training up to Masters PG Certificate level.
