Job description

NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK employing around 40,000 staff in a wide range of clinical and non-clinical professions and job roles. We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.

This is a fixed term, full time position of 9 months.

The shift pattern for this position is Monday- Friday 9am-5pm.

This is an exciting opportunity for a Nursing, AHP or Pharmacist to become part of the community element of the Home First Response Team as an advanced frailty practitioner within Renfrewshire HSCP

The post is full-time although part time options may be considered for a period of 9 months to provide resilience to the service. This is being offered as a secondment opportunity and as such applicants require agreement from their current line manager prior to application.

This post is set within a multidisciplinary team, supporting those with frailty to maximise their independence and wellbeing within their local communities and preventing avoidable hospital admissions.

We welcome applications from experienced and innovative practitioners with an appetite for service improvement and self-development, from across the NMAHP and pharmacy workforce. This specialist Band 7 post will work across the primary secondary interface, and join a team of Frailty Practitioners aligned within acute front doors and in each HSCP area. Critical to the success of this roles will be facilitating and enabling communication between and within multi-disciplinary teams across acute and community services. The frailty practitioners work as a virtual network and enable high level joint clinical decision making, with regard to right care, right place and right time to maximise patient outcomes, preventing avoidable hospital presentation and admission, and supporting discharge without delays.

Given the autonomous and specialist nature of these post we are committed to supporting and funding successful candidates to undertake post graduate training to Masters PG Certificate level.
