Job Description:

Band 7 - H@H - Advanced Nurse Practitoner (ANP / ACP)

We invite high calibre applicants to embrace this exciting opportunity to join the dynamic and enthusiastic multidisciplinary team at the Aberdeen City Hospital at Home service as we continue with planned expansion, in the role of Advanced Nurse Practitioner/Advanced Clinical Practitioner.

Hospital at Home provides short duration episodes of high quality, hospital-level acute care, to patients within their own homes, avoiding or reducing duration of hospital admissions, and supporting the wider service to improve healthcare capacity, resilience and patient outcomes. Presently, we are based at Woodend Hospital, and provide clinical cover 7 days a week between 0800-2015, across the City.

Excellent professional leadership, team working across professional boundaries, advanced clinical assessment and decision-making skills are all key to this role, and the post holder will be expected to have experience of supporting the professional development of others, including teaching, clinical supervision, and the application of research and audit, underpinned by a strong knowledge of current issues.

You will be flexible to the needs of the role and service, and will support the Leadership Team in developing the service to assure safe, patient-centred and effective care delivery, applying the 4 pillars of practice at an advanced level.

The successful applicant:

1) Will have current registration with their relevant healthcare professional body;

2) Must be in possession of a minimum of a Post-Graduate Diploma (Masters level) in Advanced Clinical Practice, or be credentialed with evidence of education and considerable clinical experience of working autonomously at Masters level;

3) Will be a qualified and experienced Independent Prescriber (V300);

4) Will have significant post-registration experience across a variety of clinical settings, ideally encompassing specialties such as Acute Medicine, Frailty or Respiratory.

Ability and willingness to travel throughout the Aberdeen City area is essential for this post. The candidate must therefore own or have access to a car for work purposes, and hold a full UK driving licence.
