Support Worker - Discharge to Assess

Location: Fife, community based

Salary: £22,932 (£12.60 per hour) per annum, pro rata

Hours: 16, 24, and 35 hours available

Contract: Permanent

This is a regulated service, and successful candidates will be required to register with the Scottish Social Services Council (SSSC) within 3 months of starting.

What will a day in the life of a Support Worker involve?
  1. Work closely with people, their families, and other professionals to create care plans tailored to their needs and goals.
  2. Assist and enable individuals with activities of daily living, including bathing, dressing, meal preparation, medication management, and mobility support.
  3. Collaborate with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
  4. Monitor and evaluate individuals' progress, adjusting care plans as needed to promote optimal recovery and well-being.
  5. Maintain accurate and thorough documentation of interactions, care plans, and progress.
To be a successful Support Worker, what will you need?
  1. Previous experience in a similar care setting, preferably within healthcare or social care, but full training will be provided.
  2. Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals' lives.
  3. Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
  4. Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
  5. Ability to work independently, demonstrating initiative and reliability.
  6. Flexibility to work various shifts, including evenings, weekends, and bank holidays.
