Job description

We are recruiting for a Dual Diagnosis Homeless Outreach Practitioner.

This is an exciting opportunity to join the award-winning START Homeless Outreach Teams as an advanced practitioner, extending our service into Lewisham.

This borough-based clinical role focuses on providing a new pathway for individuals experiencing rough sleeping, mental ill health, and substance use. It is part of an ongoing pilot in Lewisham aimed at improving access to care for homeless clients, gathering data on service use and challenges, and supporting existing agencies working with this group in the borough.

Key Responsibilities
  1. Provide expertise and contribute to dual diagnosis within evidence-based practice for people rough sleeping in Lewisham.
  2. Offer specialist assessment and clinical formulation to clients with complex mental health needs and co-occurring substance misuse, using a trauma-informed outreach approach.
  3. Conduct outreach clinics at the 999 Club and other agreed settings.
  4. Provide complex case management, addressing safeguarding for a small group of individuals who cannot be supported solely by third sector, CMHT, or CGL services.
  5. Lead the development of a seamless pathway into care for homeless service users, collaborating with local homeless, mental health, and substance use services.
  6. Work autonomously within professional guidelines, taking full responsibility for your work.
  7. Facilitate involvement of carers/family members in assessment and care planning where appropriate, and provide information about support groups.
  8. Participate in monitoring and reviewing serious incidents involving homeless dual diagnosis cases.
  9. Maintain good relationships and clear communication with other professional agencies, providing advice and support as needed.
