An exciting opportunity has arisen for a community rehabilitation band 6 physiotherapist to join a supportive and enthusiastic community therapy service as a Physiotherapist, working across community delivering rehabilitation in patients' own homes following discharge from an acute bed, or supporting long-term condition management.
This is a maternity cover for 12 months.
The team comprises qualified physiotherapists, assistant practitioners, and assistant therapy staff, who work closely with the wider MDT and INT teams in the community.
We have appointed a new Trainee Advanced Clinical Practitioner in community, and there is an opportunity to develop an interest in frailty and long-term conditions.
You will be working with patients with a wide variety of conditions and disabilities, in their own homes and local community. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff, and provide support and expertise to other team members.
We offer a comprehensive training programme within a supportive team.
The service operates across 6 days a week.

The post holder is an autonomous practitioner, responsible for specialised assessment and treatment of patients referred to the service. You will manage a caseload of patients with complex conditions, including long-term condition management and acute hospital discharges. You will see patients in their own homes, working with an MDT in other services across East Lancashire and other organisations. You will carry out specialist assessments and treatments, including prescribing essential equipment as part of the therapeutic intervention.
You will also act as a key link with PCN and MDT teams, attending meetings as part of neighbourhood team working.
The service operates Monday to Friday.
