Mersey Care NHS Foundation Trust is seeking a District Nurse Team Leader to operationally manage a District Nursing Team, ensuring quality, safety and high standards of care for patients in the community. The post involves leadership, staff development and cross-functional collaboration to drive service improvements.

You will work within a flexible, patient-centred model across community settings, leading on complex cases, safeguarding, and adherence to The Code and local policies.

Qualifications

• Experience leading community nursing teams and managing complex caseloads.
• Strong clinical governance knowledge and adherence to NICE guidelines.
• Ability to develop care plans and drive service improvements.

Responsabilités

• Lead a District Nursing Team and ensure quality, safety and patient outcomes.
• Manage and co-ordinate long-term condition care and prevention strategies.
• Develop staff through appraisals, PDPs and professional development.
• Collaborate with GPs, nurses, and other services to ensure integrated care.
• Promote patient-centred care and effective communication within the team.
