The Powys Teaching Health Board is seeking a District Nurse Deputy Team Leader to join our community nursing service in Newtown. This Band 6/Annex 21 role requires delivering autonomous district nursing care in patients’ homes and leading a multi-disciplinary team.

You will work across 7 days with shifts between 8am and 8pm, fostering professional development and supporting services to Start Well, Live Well and Age Well.

Qualifications

• NMC Registered Nurse with current registration.
• Post-registration District Nursing qualification (BSc in relevant subject).
• Experience in proactive caseload management of wide range of patients including chronic conditions.
• Annex 21 Band 6: working toward DN SPQ qualification.
• MSc Advanced Clinical Practice and leadership preparation.

Responsabilités

• Deliver autonomous nursing care to patients in their homes within a multi-disciplinary team.
• Provide palliative and end-of-life care and assess carers and patients with long-term conditions.
• Promote self-management, wound care and chronic disease management.
• Travel across the locality; lone-working ability essential.
• Support development and service improvements within the team.
