Job summary

Lincolnshire Community Health Services NHS Trust is seeking a full-time District Nurse/Case Manager to join the Gainsborough Community Nursing Team. Applicants should hold the Community Specialist Practitioner qualification and have experience managing a caseload and leading a team.

There is a fixed-term post for 1 year to cover a Case Manager role while competencies are obtained. Shift pattern 8:00–18:30, 7 days service.

Qualifications

• Experience in NHS or related multi-professional setting
• Significant post-registration clinical experience
• Experience of working autonomously in a senior position
• Evidence of computer literacy and knowledge of Microsoft applications
• Evidence of extended clinical skills pertinent to role

Responsibilities

• Travel to patients' homes and clinic settings as required in a community post
• Coordinate care delivery for caseload of patients
• Lead and support the community nursing team through service transformation
