Job summary

Lincolnshire Community Health Services NHS Trust is seeking an experienced District Nurse for a full-time, 12-month fixed-term post to cover a Case Manager who is training. You will manage a caseload, coordinate care, and lead the community nursing team through service changes.

The role requires Community Specialist Practitioner qualification and senior autonomous experience, with seven-day service and transport access. Development route to Case Manager is offered.

Qualifications

Job description

A full-time District Nurse post within the Horncastle and Spilsby ICT at Lincolnshire Community Health Services NHS Trust, on a 12-month fixed-term basis to cover a Case Manager who is training. The post holder manages a caseload of patients with varied needs, coordinates their care and leads the community nursing team through service change. Care runs across community settings on a seven-day service from 8:00 to 18:30 using shift work, and the role requires access to transport. Candidates need the Community Specialist Practitioner qualification and senior autonomous experience.

• Evidence of extended clinical skills relevant to the role
