Job summary

NHS Greater Glasgow and Clyde is seeking a District Nurse Specialist Practitioner to lead and deliver high-quality, person-centred care in patients' homes and communities from Clydebank Health Centre. You will work autonomously within a multidisciplinary team, supervise staff, ensure safe practice, maintain records, and contribute to service development, while promoting flexible working options.

The role requires an NMC registered Adult Nurse with a District Nursing Master’s qualification,

Qualifications

  • Registered Nurse with current NMC registration in Adult Nursing.
  • Specialist Practitioner Qualification in District Nursing at Master's level or equivalent.
  • V300 Independent Prescribing qualification and advanced clinical assessment skills.
  • Demonstrable leadership and management experience within a community or primary care setting.
  • Evidence of continuous professional development and commitment to lifelong learning.
  • Excellent communication, problem-solving and organisational skills with the ability to work autonomously and within a team.
  • IT skills and ability to use electronic patient record systems alongside a full driving licence and access to a vehicle.

Responsibilities

  • Lead and manage a defined district nursing caseload, ensuring safe, effective, person-centred care delivery.
  • Provide skilled clinical interventions and act as the clinical expert within community nursing practice.
  • Coordinate workload, prioritise care needs and respond to both planned and urgent unscheduled requests.
  • Work collaboratively with multidisciplinary teams, patients and carers to plan and deliver holistic care.
  • Delegate, supervise and support team members to maintain high standards of practice and service delivery.
  • Maintain accurate clinical records and ensure compliance with professional, legal and organisational standards.
  • Contribute to service improvement, audit, education and the ongoing development of the district nursing team.

The role:

An exciting opportunity has arisen within the West Dunbartonshire District Nursing Team for a District Nurse Specialist Practitioner working within NHS Greater Glasgow and Clyde. This is a rewarding and dynamic role where you will deliver high-quality, person-centred care to patients in their own homes and communities. As an autonomous practitioner, you will lead and support the district nursing team, ensuring safe, responsive and effective care across a diverse caseload. You will work closely with multidisciplinary partners to coordinate care and improve patient outcomes. Duration, Location, and Working Pattern:
  • Full Time, Permanent
  • Base Location: Clydebank Health Centre
  • Shift Pattern: Monday to Sunday, 4 x 9 hours
Key responsibilities:

Knowledge, training, qualifications and/or experience required to do the job:
  • Registered Nurse with current NMC registration in Adult Nursing

  • V300 Independent Prescribing qualification and advanced clinical assessment skills
  • Demonstrable leadership and management experience within a community or primary care setting
