Buckinghamshire Healthcare NHS Trust in Amersham is seeking a part-time Community Case Manager/District Nurse to join the Amersham Adult Community Healthcare Team. The post involves home visits, nursing interventions, and MDT work, with a 7am–9pm service 7 days a week.

Qualifikationen

• Essential criteria: Registered Allied Healthcare Professional / Registered Level 1
• Essential criteria: Nurse Degree/ Diploma in Health / Nursing Studies or related subject
• Evidence of recent Continuous Professional Development
• At least one year experience as a Band 5 in the community
• Post graduate clinical training in Elderly, Long Term Conditions, Respiratory, Cardiac, Dementia, Rehabilitation, Trauma and Orthopaedics, End of Life

Aufgaben

• The case management model will provide interventions tailored to meet the needs of the individual
• Contribute to processes for caseload management for patients at high risk of deterioration and unplanned admission
• Assess physical and psycho-social needs and instigate therapeutic treatments and care delivery based on evidence
• Improve health outcomes using specialist nursing skills
• Undertake referral management to develop case management programmes for Adults
• Manage a caseload of patients with long term conditions who would benefit from case management
