Job summary

British Red Cross in Scotland is seeking a Senior Practitioner to lead discharge-to-assess services, guiding staff to deliver person-centred care as people move from hospital to home.

You'll coordinate referrals with hospitals and GP surgeries, mentor students and volunteers, conduct risk assessments, manage rotas, and ensure continuity of high-quality support.

Qualifications

• SVQ Level 3 in Health and Social Care and PDA in Supervision (or equivalent).
• Strong communication and leadership skills.
• Experience in person-centred planning and multi-disciplinary working.
• Ability to mentor staff and support ongoing development.

Responsibilities

Senior Practitioner - Discharge to Assess

Location: South Ayrshire
Salary: £25,541 to £27,200 per annum
Hours: 35 per week
Contract: Permanent

Additional Requirement: Full UK driving licence required and access to a vehicle - business milage paid

As a Senior Practitioner, you’ll play a key part in delivering high-quality care and support within a regulated service. Working closely with the Service Manager, you’ll ensure service users receive compassionate, person-centred support while guiding and mentoring staff to grow in their roles. What will a day in the life of a Senior Practitioner involve?

To be a successful Senior Practitioner, what will you need?

• SVQ Level 3 in Health and Social Care and PDA in Supervision (or equivalent).
• Strong communication skills and the ability to lead and motivate others.
• Experience in person-centred planning and multi-disciplinary working.
• Confidence in mentoring staff and supporting their ongoing development.
