We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.
The shift pattern for this position is Mon-Fri 0830am-1630 pm and the post will be NHS Great Glasgow & Clyde board wide.
(Please note the salary is Pro Rata) for part time hours.
Working as part of the Discharge Team you will provide specialist discharge advice and intervention to all staff, patients, relatives and carers for all directorates within the Acute Services Division. The Discharge Team also includes Discharge Lounges and the Acute Homeless Liaison Team. The Discharge Team is a board wide service within the Acute Adult General Hospitals.
A first level registered adult nurse on the NMC register with significant post registration experience in acute and/or community setting. Applicants must have band 6 experience. Proven management experience as well as an excellent working knowledge of discharge planning in acute care is essential.
