Job summary

Imperial College Healthcare NHS Trust is seeking an experienced and motivated individual to join our complex discharge team, predominantly based at Charing Cross Hospital but working across sites as required. You will manage a caseload of patients with complex needs and contribute to education of peers within the Trust.

The role requires strong communication, initiative, and teamwork, with weekend and bank holiday service cover as part of contracted hours.

Qualifications

• Excellent communication and interpersonal skills.

Responsibilities

• Deliver high standards of patient care and pathway management to reduce delayed transfers of care across the trust.
• Coordinate assessment of patients’ needs and ensure safe discharge planning.
• Act as SMH Discharge Hub screener on a rotational basis.
• Deputise for the Discharge Team Manager at Trust meetings as required.
• Support staff education and contribute to development of training opportunities.
