Job summary

East of England Community Health & Care NHS Trust is seeking a Band 6 Liaison Sister/Charge Nurse to lead discharge planning across hospital and community settings in the Luton area. Qualifications

• Current registration with the NMC as a nurse.
• Experience as a Band 6 sister/charge nurse or equivalent.
• Experience in integrated working and discharge planning.

Responsibilities

• Work with hospital and community teams to ensure safe discharge, including End-of-life and Fast Track Funding patients.
• Facilitate clear communication between patients, families, carers and professionals to ensure seamless care.
• Enable patients and families to participate in discharge planning, encouraging independence and choice.
• Prescribe equipment as needed to promote patient safety and enable safe community care.
• Manage GP Liaison service by handling referrals, basic triage, and signposting.
• Provide clinical supervision and 1:1 support to team members.
• Recognise cross-boundary challenges in complex cases and coordinate pathways.
