Job summary

Maidstone Hospital is seeking a compassionate and proactive individual to join the Integrated Discharge Team, helping ensure patients can leave hospital safely and promptly by identifying and resolving barriers to discharge.

You will work closely with patients, families, healthcare professionals and community services to support transitions from hospital to home, including home visits to assess discharge needs.
