Berkshire Healthcare NHS Foundation Trust's Royal Berkshire Hospital is seeking a Discharge Facilitator to coordinate safe, timely moves from hospital to the next care setting. You will work with hospital and community partners to improve patient flow and ensure plans are progressed efficiently.

A proactive, organised candidate with excellent communication and problem-solving skills will thrive in a fast-paced environment, balance priorities, and contribute to Quality Improvement initiatives.

Responsibilities

• Coordinate timely discharges and plan patient moves to the next setting.
• Monitor discharge activity and provide performance reports.
• Collaborate with clinical teams and community partners to progress plans.
• Participate in Saturday working as part of the service rota.
