We are looking for a Clinical Discharge Coordinator to complement our existing team.
The right individual should have knowledge and experience of discharges from acute and community hospitals, knowledge and experience of the mental capacity Act and an understanding of Continuing Healthcare and Social Care Act.
A general overview of responsibilities includes providing support to and working extensively with multi-disciplinary and multi-agency staff to co-ordinate discharge planning for those patients who require on-going care following discharge from hospital.
Candidates will need to have excellent organisational and negotiation skills and a detailed knowledge of care needs documentation, e.g. health needs assessments, continuing health care assessments, continuing care checklists, decision support tools and mental capacity assessments.
You will also need to have excellent communication skills which will enable you to work sensitively with patients and families at what can be a very difficult time in their lives and have an ability to problem solve and an ability to work under pressure.
