Job summary

HCRG Care Group is seeking a Medical Lead to provide strategic clinical leadership across inpatient community units in Swindon and North Wiltshire. You will drive safe, person-centred care for frail older adults, oversee medical colleagues, and support service development through job planning and appraisals.

Reporting to the Medical Director, BSW, you will lead multidisciplinary teams, optimise patient pathways, and ensure timely discharge.

Qualifications

• Full GMC registration and licence to practise.
• Significant post-registration clinical experience, including care of older adults.
• Strong experience in multidisciplinary teams and care integration.
• Commitment to CPD and professional development.
• Solid understanding of clinical governance, patient safety and quality improvement.
• Experience contributing to audit, research or service improvement initiatives.
• Excellent communication and interpersonal skills.
• Knowledge of current healthcare policy and service developments.

Job Introduction

We are seeking an experienced and compassionate Medical Leadto provide clinical leadership and expert medical care across our inpatient community units in Swindon and North Wiltshire. This is an exciting opportunity to shape the future of integrated care services for older, frail patients with complex health needs, while supporting the development of a high-performing multidisciplinary team.

Reporting to the Medical Director, BSW, you will play a key leadership role within HCRG Care Group, driving excellence in patient care, supporting medical colleagues through job planning and appraisals, and contributing to the strategic development of our inpatient services.

About the Role

As Medical Lead, you will provide medical oversight and care for patients within our inpatient community units, focusing on older adults living with frailty, multimorbidity, and advanced clinical needs. Working closely with multidisciplinary teams, you will help deliver safe, effective, person-centred care that supports recovery, rehabilitation, and timely discharge.

You will be based predominantly across:

• Swindon Intermediate Care Centre - 60 rehabilitation and stroke beds across two wards.

Most patients are transferred from Great Western Hospital as a step-down from acute care, with others admitted directly from community services. This vital role helps bridge the gap between hospital and community-based care, ensuring the best possible outcomes for patients.

Key Responsibilities

Working within a multidisciplinary team of Advanced Practitioners, Consultant Practitioners and therapists, you will provide expert clinical leadership and care for patients with urgent, complex and frailty-related healthcare needs. You will play a key role in assessing, diagnosing and managing patients referred through the Urgent Community Response service and wider clinical pathways, supporting individuals who would otherwise require hospital admission or who need timely discharge from acute care.

Working closely with Care Coordination, Flow Teams, acute hospitals and 111/999 services, you will help develop and improve patient pathways, maximising the use of community services, Hospital at Home and inpatient capacity. You will be responsible for coordinating high-quality, person-centred care across primary, community, acute and social care settings, while maintaining clinical accountability throughout the patient journey and delivering the best possible outcomes for patients with complex healthcare needs.

• Providing expert clinical leadership and oversight for patients with urgent, complex and frailty-related healthcare needs across community and inpatient services.
• Delivering high-quality, evidence-based assessment, diagnosis, treatment planning and ongoing management, ensuring safe and effective patient care throughout the care pathway.
• Working collaboratively with multidisciplinary teams, including Advanced Practitioners, Consultant Practitioners, therapists and wider healthcare professionals, to achieve the best possible outcomes for patients.
• Supporting the development and optimisation of patient pathways into, through and out of services, helping to reduce unnecessary hospital admissions and facilitate timely discharge from acute settings.
• Partnering with Care Coordination, Flow Teams, 111/999 services and acute hospitals to maximise the use of community services, Hospital at Home and inpatient capacity.
• Leading the coordination of care for patients who would otherwise require hospital admission, ensuring seamless transitions across primary, community, acute and social care services.
• Maintaining clinical accountability for patient caseloads, ensuring appropriate escalation, intervention and management of deteriorating patients.
• Prescribing and review medications in line with clinical need, working closely with pharmacy colleagues to support safe medicines management and discharge planning.
• Acting as a visible clinical leader and role model, promoting excellence, quality and patient safety while supporting collaborative and innovative ways of working.
• Leading appraisals and contribute to the job planning and professional development of medical colleagues within the inpatient units.
• Driving continuous improvement through clinical audit, research, quality improvement initiatives and the implementation of evidence-based practice.
• Fostering a positive learning culture by supporting education, training and development opportunities for colleagues and students.
• Championng a culture of reflection, learning and improvement, identifying opportunities to enhance services and improve patient outcomes.

What We're Looking For

• Full GMC registration with a current licence to practise.
• Significant post-registration clinical experience, including the assessment and management of patients in community and/or inpatient settings.
• Strong experience caring for older adults, frail patients and those with long-term or complex health conditions.
• A proven ability to work effectively within multidisciplinary teams, building strong relationships across services.
• Commitment to continuing professional development (CPD) and maintaining up-to-date clinical knowledge.
• A sound understanding of clinical governance, patient safety and quality improvement.
• Experience contributing to audit, research or service improvement initiatives that enhance patient care.
• Excellent communication and interpersonal skills, with the ability to engage effectively with patients, families and colleagues.
• Knowledge of current healthcare policy, service developments and innovative approaches to delivering care closer to home.
• Previous leadership, management or quality improvement experience would be advantageous, as would qualifications such as MRCP, MRCGP, Diploma in Geriatric Medicine or a relevant higher degree.
