Job summary

Southport and Formby Health (SAFH) are seeking a Digital Transformation Care Coordinator to support the PCN digital agenda. The role combines office-based work at the Southport site with visits to the fourteen practices, coordinating EMIS and digital systems, and driving service improvements.

You will work with clinical and non-clinical colleagues, analyse data, support Health Inequality Reduction, and help implement digital health solutions across practices.

Qualifications

• Educated to GCSE or A Level in English and Maths.
• Experience in healthcare settings preferred.
• Digitally proficient with ability to work in a busy team.

Responsibilities

• Support EMIS and Digital Systems within the PCN and practices.
• Collaborate with PCN management to drive digital transformation and service improvements.
• Assist practices with digital health tools and data reporting.
• Support health inclusion and reducing health inequalities through digital means.
• Manage projects and track progress across digital initiatives.
