Job summary

Operose Health is hiring a registered pharmacist to support primary care delivery at scale. You will work with clinicians to improve patient care, run telephone triage clinics, and manage prescribing decisions.

The role includes reviewing hospital correspondence and coordinating medicines with local pharmacies. The ideal candidate holds GPhC registration, excels in communication, and is committed to improving patient outcomes within a collaborative team.

Qualifications

• Excellent communication skills both verbally and written.
• Ability to work well in a team and develop clinically.
• Proactive approach to improving patient care.

Responsibilities

• Collaborate with clinicians to improve patient care and support prescribing decisions.
• Run daily telephone triage clinics and triage pathology results with patients.
• Review hospital correspondence and coordinate medicine reconciliation or initiation with local pharmacies.
• Lead repeat prescription service and participate in home visits and immunisation initiatives.
