An exciting opportunity has arisen in the established Diabetes Nursing team!
This role is a permanent position providing care for people within the community diabetes service. The role will be based within the community but working across an integrated nursing service, utilizing experience and specialised knowledge to provide education, advice, initiation, and adjustment to treatment plans.

The community diabetes specialist nurse will assist in delivering a specialist clinical diabetes service aimed at providing high-quality diabetes care within Swindon, aligned with the service and Trust objectives.
They will also deliver high-quality education to practice nurses and GPs to enhance diabetes care in primary care settings.
