Join the Leeds Community Diabetes Service as a diabetes specialist nurse. This exciting opportunity includes assessing patients, implementing treatment plans, and providing education on diabetes management. You will be part of a dedicated multidisciplinary team committed to delivering high-quality diabetes care and supporting your professional development.
Are you a band 5 or current band 6 nurse with an interest in diabetes who wants to expand your knowledge and skills in diabetes care and management?
If so, the Leeds Community Diabetes Service in Leeds has a role for you in the team.
We have an exciting opportunity for a diabetes specialist nurse to join our multidisciplinary team, consisting of nurses, dietitians, GPs, diabetes facilitators, and pharmacists. If successful, you would work across Leeds providing face-to-face and telephone clinics, conducting home visits, and leading education groups for patients with type 2 diabetes.
This role is suitable for someone enthusiastic and committed to delivering excellent diabetes care and eager to develop further expertise in this specialty. Our team is dedicated to staff wellbeing and development, and the successful candidate will be supported to complete NMP training if not already achieved, as this is an essential requirement for the role.
The role requires excellent communication skills as you will undertake complex assessments of patients with diabetes, implement treatment plans, and review and assess progress, all while promoting self-management for individuals with diabetes.
Having a car or suitable alternative transport is essential for this role, as public transport is unlikely to meet this requirement.
