Job summary

Derbyshire Community Health Services NHS Foundation Trust (DCHS) is seeking a Specialist Community Diabetes Nurse to join the Diabetes Nursing team. The post involves comprehensive assessments, creating individualized treatment plans with patients, and collaborating with GPs and consultants across North Derbyshire.

You will support education for patients and professionals, maintain competency through CPD activities, and travel across the locality to deliver care in community settings.

Qualifications

  • Registered General Nurse or RMN (or equivalent) with post-registration diabetes training
  • Good understanding of the national diabetes agenda, prevention and treatment
  • Evidence of working clinically at a senior level
  • Community experience
  • Non-Medical Prescriber or willingness to undertake relevant training

Responsibilities

  • Perform comprehensive nursing and diabetes assessment with/for patients
  • Formulate a diagnosis and develop, plan and deliver an effective individualized treatment programme in partnership with patients
  • Continually evaluate patient progress, reassess and amend treatment as appropriate as part of an individualized programme
  • Manage assessment and clinical risk within own caseload
  • Be professionally and legally accountable for all aspects of professional and clinical work
  • Communicate verbal and written reports and discharge summaries to other agencies as appropriate
  • Communicate effectively with patients to gain informed consent and agree outcomes
  • Review medication for therapeutic effectiveness in line with protocols – non-medical prescriber (for band 7)
  • Motivate patients and carers to enable adherence
  • Work with patients to develop self-management and care planning
  • Deliver training to patients and professionals on diabetes care
  • Explain complex information to patients in an understandable form
  • Teach MDT members, patients and carers how to use diabetes equipment

Job overview

The post holder will work as part of the Specialist Community Diabetes Nursing team within DCHS to:

  • Provide a comprehensive service to adult patients with diabetes referred to the service
  • Work in collaboration with GP’s and consultants within North Derbyshire area
  • Work with other members of the DSN team to support the delivery of community based education to individuals and groups within North Derbyshire to promote high quality support for adults with diabetes
  • Work as part of the team to deliver training to a range of professionals to support with their care of people with diabetes

Be responsible for maintaining competency to practice through maintenance of professional registration, continuous professional development activities and maintain a portfolio of evidence of learning beyond registration.

Main duties of the job
  • To perform comprehensive nursing and diabetes assessment with/for patients.
  • To be responsible for formulating a diagnosis and developing, planning and delivering an effective individualised treatment programme in partnership with patients.
  • To continually evaluate patient progress, reassess and amended treatment as appropriate as part of an individualised treatment programme in partnership with patients
  • To be responsible for assessment and management of clinical risk within own case load
  • To be professionally and legally accountable for all aspects of professional and clinical work
  • To communicate in timely verbal and written reports and discharge summaries to other agencies as appropriate
  • To communicate effectively with patients, to gain informed consent to treatment and agree expected outcomes.
  • Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national/local protocols – non-medical prescriber (for band 7).
  • Motivate patients and where appropriate carers to enable agreed treatment programmes are adhered to
  • To work with patients to develop a self-management approach to care and care planning
  • To participate in the delivery of training to both patients and other professionals on a 1:1 and group basis
  • To communicate complex and sensitive information in an understandable form to patients
  • To teach members of the multi-disciplinary team , patients and carers how to use diabetes equipment

Qualifications and ability to answer questions at interview
