Diabetes Specialist Nurse

This is an exciting opportunity to work as our lead nurse for the inpatient diabetes team at Countess of Chester NHS Trust. This is an ideal opportunity for a current band 7 nurse to continue their development, or for a band 6 nurse with inpatient experience wanting to expand their knowledge in the area, to shape the inpatient diabetes experience for patients, families, and staff at the Countess.

It is an opportunity for someone with enthusiasm for inpatient diabetes and a wish to extend their expertise in this field. The post will include responsibility for leading the service and planning the future care for inpatients with diabetes, using GRIFT and JBDS guidelines to push the service forward. You will be involved in developing and shaping our service working within the MDT diabetes team and wider hospital specialties so that it is fit for the future for our patients with diabetes while they are residing at the Countess of Chester. You will need to show empathy and sensitivity to the needs of the patients, have good communication skills, together with a friendly, polite, and professional attitude.

You will need excellent clinical, organizational, IT, and communication skills.

Main duties of the job
  1. Work as a highly-skilled independent practitioner implementing and recommending treatment based on expert clinical knowledge.
  2. Provide comprehensive evidence-based highly specialist clinical care, training, and consultancy to both people with diabetes and other professionals.
  3. Provide advice, support, and education to patients and their families.
  4. Contribute to MDT meetings/case conferences, referrals to other services to meet patient needs.
  5. Assess, advise, and take appropriate action for bleeps, ward referrals, emergency and routine phone calls from GPs, practice nurses, district nurses, hospital-based staff, and other healthcare professionals.
  6. Manage the inpatient caseload of patients with multiple medical conditions and complex needs.
  7. Provide an expert diabetes nursing service, offering support, advice, and information.
  8. Act as an expert resource, providing clinical advice and support to others involved in caring for people with diabetes, in secondary care and community settings.
  9. Work as an independent practitioner, demonstrating specialist skills and knowledge through expert nursing.
  10. Practice advanced clinical skills and maintain a high standard of care and expertise.
About us

The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and Welsh patients covered by Betsi Cadwaladr University Health Board. The Trust works collaboratively within the wider Cheshire and Merseyside Health and Care Partnership. Its services are provided from three locations:

  • The Countess of Chester Hospital: providing 438 general and acute beds.
  • Ellesmere Port Hospital: providing 56 beds as a rehabilitation, intermediate, and outpatient facility.
  • Tarporley War Memorial Hospital: a base for community services serving the local rural population.

The Trust employs over 6,258 staff, including temporary bank staff, providing acute emergency and elective services, primary care, direct access services, and obstetric services to a population of approximately 407,000, including residents in Chester, West Cheshire, Ellesmere Port, Neston, and parts of Flintshire.

In 2022/2023, there were over 503,342 patient attendances, ranging from outpatient appointments to major cancer surgeries, an increase of nearly 10,000 from the previous year.

Person Specification
Qualifications
  • Registered Nurse, RGN/RN (Adult), with a diploma or degree in nursing or equivalent experience.
  • Registered Nurse, R Nurse Prescriber, or working towards this qualification.
  • Evidence of Continuing Professional Development, including post-registration training/experience in diabetes.
  • Post-registration Diabetes Qualification.
  • MSc in a relevant topic.
  • Teaching qualification.
Knowledge
  • Experience in service development and implementing change.
  • Substantial post-registration experience, including specialist diabetes experience.
  • Experience in developing and delivering teaching programs.
  • Significant previous experience as a Diabetes Nurse.
Skills
  • Advanced communication skills to influence and overcome resistance.
  • Ability to present information to professional groups.
  • Ability to supervise and support junior team members.
  • Commitment to improving services.
  • Ability to work independently and as part of a multidisciplinary team.
