• Implement and evaluate individual treatment plans for patients with long-term conditions.
• Perform appropriate tests for diagnosing and reviewing long-term conditions (e.g., diabetes).
• Identify and manage treatment plans for patients at risk of developing a long-term condition.
• Prioritise health problems and intervene in urgent or emergency situations, including initiating emergency care.
• Provide information on medications, side effects and interactions; advise on regimens.
