Job summary

Queen Square Medical Practice in Lancaster is seeking an experienced Practice Nurse with a focus on diabetes management. You will lead and support diabetes care within the team, undertake comprehensive reviews, and help achieve QoF targets while delivering education and long-term condition management.

The successful candidate will have substantial general practice nursing experience, support for professional development, and may have independent prescribing capabilities.

Qualifications

• Extensive experience in diabetes care within a general practice setting.
• Ability to lead diabetes reviews and contribute to QoF targets.
• Experience in managing long-term conditions and delivering patient education.

Responsibilities

• Lead and support delivery of diabetes care within the practice.
• Undertake comprehensive diabetes reviews and annual assessments.
• Interpret investigations and act on results; titrate insulin where qualified.
• Deliver patient education to promote self-management and improve outcomes.
• Identify and proactively manage patients at risk of complications.
