The post holder will work closely with and deputise for the ward manager in their absence. Be an active member of the multi-disciplinary team, supervise, teach, and instruct junior staff, Nursing Assistants, Activity Coordinators, and STR Workers in assessing, planning, implementing, and evaluating patient care.

Working independently, the post holder will be responsible for developing and coordinating the individual’s assessment and care plan as agreed with the multidisciplinary team. The post holder will adhere to Trust policies, data protection, and Health and Safety standards.

Responsibilities include:
  1. Participate as an effective MDT member, developing and implementing a patient-centered approach to care, assessing, developing, implementing, and evaluating patient and carer needs.
  2. Interpret clinical observations, act upon them, and communicate information to team members following clinical protocols.
  3. Determine nursing priorities, plan patient care, conduct risk assessments, and inform the Ward manager of any changes affecting care standards.
  4. Meet the physical, psychological, social, and cultural needs of individuals and their families, using research-based knowledge to maintain their identity.
  5. Recognize family and carer needs, involve them in care, and offer support and education with empathy.
  6. Maintain high standards of nursing care, participate in setting and reviewing standards, nursing audits, and support staff in delivering quality care.
