We are currently undergoing a transformation within our service as part of the National Transformations of community services and the local Living Well developments. It is an opportunity to be part of our service development, with planned investment in our staff and new role development.
The CMHT work with people aged 16 and over with severe and enduring mental health and social care needs.
In this role you will be responsible for supporting the CMHT team manager in managing and coordinating the Community Mental Health Team and ensuring that the team works cohesively and meets its objectives in the most effective and efficient way,
The post-holder will act as a named worker for a reduced caseload of mental health service users who have complex needs.
