Job summary

Pennine Care is seeking a Community Mental Health Practitioner to provide holistic assessments, interventions, and care planning for adults with severe and enduring mental health needs in Stockport. You will act as a key worker for a defined caseload and supervise junior staff and students within a multidisciplinary team.

The role involves administering depot medication for individuals unable to attend the depot clinic, with a focus on recovery and social inclusion.

Qualifications

  • Professional qualification in Social Work, Mental Health Nursing, or Occupational Therapy.
  • Registration with HCPC or NMC is required.
  • Experience in community mental health work is essential.
  • Willingness to undertake further training due to legislative or service changes.

An opportunity has arisen for a Community Mental Health Practitioner looking to join a great team, providing specialist mental health care to the population of Stockport. The Secondary Care CMHT support residents with severe and enduring mental health conditions. population we serve.

This an exciting time for Stockport CMHT. We are currently undergoing a transformation within our service as part of the National Transformations of community services and the local Living Well developments. It is an opportunity to be part of our service development, with planned investment in our staff and new role development.

The CMHT work with people aged 16 and over with severe and enduring mental health and social care needs.

Mental Health Practitioners are responsible for providing holistic assessment, interventions, risk assessments and care plans for people on their caseload. As a nurse in the team. You will be responsible for administering depot medication for people who are unable to attend the depot clinic.

Main duties of the job

To act as the community Mental Health Practitioner and key worker responsible for the care of an identified group of clients, being part of a service that is committed to recovery and social inclusion.
To provide supervision to an identified group of staff and students on placement with the team.

Education/Qualifications
  • Professional qualification Social Work / Mental Health Nursing / Occupational Therapy
  • Registration with HCPC / NMC
  • Experience in community mental health work
  • Willingness to undertake further training relevant to the practitioner role resulting from legislative or service changes, as appropriate.
  • Evidence of post-basic qualification and training
  • PSI Training
  • MSLAP or equivalent Mentor training
Experience
  • Working within Mental Health Law policies and procedures.
  • Experience of supervision
  • Experience in working with people with moderate to severe mental health needs
  • Experience of MDT working
  • Experience of leading a team / ’acting up’.
Knowledge
  • Extensive knowledge / experience in social inclusion and recovery model of care.
  • Knowledge of mental health legislation
  • Experience of managing or participating in change
Skills and Abilities
  • Ability to and be responsible for management of a caseload
  • Ability to work effectively as part of a MDT.
  • Ability to supervise junior staff / students
  • Effective time management
  • Excellent written and verbal communication skills
  • Recording relevant documentation to a professional standard.
  • Ability to communicate informally and develop supportive relationships with clients, carers and other professionals
  • Utilise the supervision process.
  • Ability to value diversity and respond constructively to discriminatory behaviour

  • Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
