Job summary

Surrey and Borders Partnership NHS Foundation Trust invites applications for a Deputy Manager in the Intensive Support Service. The role supports individuals with learning disabilities and complex needs in community and inpatient settings, under the Learning Disabilities Directorate.

The post offers development within a multidisciplinary team, with joint working across disciplines and agencies, and a focus on person-centred care and safeguarding within the Positive Behaviour Support framework.

Qualifications

  • RNLD/RMN registration with valid professional body.
  • Experience delivering care to people with learning disabilities and complex needs.

Responsibilities

  • Demonstrate expertise in crisis intervention, learning disability care, epilepsy, medication management and challenging behaviour.
  • Actively involve people in developing and agreeing their own care plans, advocating dignity and cultural needs.
  • Carry out comprehensive nursing assessments and deliver holistic interventions across settings.
  • Practice within the Positive Behaviour Support framework.
  • Undertake comprehensive risk assessments and develop risk management plans.
  • Collaborate with acute and primary care services and maintain relationships with colleagues and families.

Job description

Deputy Manager Intensive Support Service- Band 6 (would consider a band 5 on a developmental role initially).

An exciting opportunity has arisen for a RNLD / RMN to join the Intensive Support Service, within the Trust's Learning Disabilities Directorate. The Intensive Support Service is an innovative team, created in response to the Transforming Care agenda. We support people with learning disabilities and complex needs, within the community and inpatient settings.

The Multi-Disciplinary Team include Nursing, Senior Support Workers, Occupational Therapy, Speech & Language Therapy, Psychology, Psychiatry, Therapy Assistants and assistant Psychologist.

The service is designed to support people whose mental health and/or behaviour support needs put them at risk of admission to hospital, or of placement breakdown. We work closely with Community Team for People with Learning Disabilities in the East and West.

Qualifications

The post will offer excellent opportunities for development and support within the community crisis team and the inpatient unit. Opportunities for joint working with other disciplines and agencies would be an integral part to the role.   • To demonstrate expertise and specialist nursing skills in areas associated with learning disability such as crisis intervention, alternative communication, the assessment and management of people with additional mental health needs, epilepsy, medication management, challenging behaviour and other conditions.
  • To actively involve people in developing and agreeing their own care plan, to act as an advocate and ensure that there is proper regard for their dignity, choice, self-esteem, racial, cultural, sexual and religious needs and preferences.
  • To carry out comprehensive assessments, plan implement and evaluate delivery of nursing interventions as part of a holistic approach working in a variety of settings.
  • To practice within the Positive Behaviour Support framework.
  • To undertake comprehensive risk assessments and develop effective risk management plans.
  • To work in partnership with acute and primary care services and other healthcare providers.
  • To establish and maintain effective relationships with colleagues, other agencies, people who use services and their families.
