Our High Peak and North Dales CMHT team has an opportunity for a Dementia Support Worker.
The Dementia Support Worker will collaborate with the High Peak Primary Care Network to provide practice-based support for Older Adults with Mental Health issues related to cognitive impairment and Dementia.

Working hours?
The team operates Monday to Friday, 9am-5pm. This role offers 37.5 hours per week.
Work location?
Base: The successful candidate will work within GP practices including Sett Valley Medical Centre, Stewart Medical Centre, Thornbrook Surgery, Buxton Medical Practice, Elmwood Medical Practice, Goyt Valley Medical Practice, Hartington Surgery, Arden House Medical Practice. The main base will be with the Older Adult Community Mental Health Team at Bath Road, Buxton.
Area Covered: Buxton and surrounding areas of Derbyshire.
Travel requirements: The role involves traveling to various community sites. Public transport may not always be suitable or fast enough for these visits.
The role involves supporting individuals with dementia, providing education, pre-assessment support, and signposting to relevant services.
As an employer and sponsor license holder, we must adhere to UK Visa and Immigration requirements. Please note, without prior NHS service, you will start at the bottom of the pay band, which may not meet the minimum salary for sponsorship via the Skilled Worker route. Salary: £24,625 to £25,674 per annum, pro-rata.
