Job description

Do you have experience working with Older Adults with Mental Health issues related to cognitive impairment and Dementia?

Our High Peak and North Dales CMHT team has an opportunity for a Dementia Support Worker.

The Dementia Support Worker will collaborate with the High Peak Primary Care Network to provide practice-based support for Older Adults with Mental Health issues related to cognitive impairment and Dementia.

We will hold a recruitment day for this position! If interested, please join us on Thursday, 12th June, for an assessment day.

Time: 10am - 12 noon (Please arrive by 9:30am for prompt start)

Venue: Deepdale Business Park, Bakewell, DE45 1GT

Main duties of the job

Working hours?

The team operates Monday to Friday, 9am-5pm. This role offers 37.5 hours per week.

Work location?

Base: The successful candidate will work within GP practices including Sett Valley Medical Centre, Stewart Medical Centre, Thornbrook Surgery, Buxton Medical Practice, Elmwood Medical Practice, Goyt Valley Medical Practice, Hartington Surgery, Arden House Medical Practice. The main base will be with the Older Adult Community Mental Health Team at Bath Road, Buxton.

Area Covered: Buxton and surrounding areas of Derbyshire.

Travel requirements: The role involves traveling to various community sites. Public transport may not always be suitable or fast enough for these visits.

The role involves supporting individuals with dementia, providing education, pre-assessment support, and signposting to relevant services.

As an employer and sponsor license holder, we must adhere to UK Visa and Immigration requirements. Please note, without prior NHS service, you will start at the bottom of the pay band, which may not meet the minimum salary for sponsorship via the Skilled Worker route.   • Supporting GP practices within the Primary Care Network
  • Spending dedicated time weekly with the CMHT, attending meetings, supervision, and training
  • Liaising between GP practices and the CMHT
  • Supporting individuals with cognitive issues and diagnosed dementia
  • Receiving supervision from the CMHT and PCN to share mental health expertise
  • Practice-based work overseen by designated GPs
Person Specification
Qualifications
  • NVQ Level 3 in Health & Social Care or equivalent
Skills
  • Ability to work as part of a team
  • Ability to work independently and recognize limitations
  • Basic IT skills
  • Willingness to travel as required
Knowledge and Experience
  • Understanding of issues faced by people with organic mental health diagnoses across all ages
  • Experience working directly with health and social care service users (paid or voluntary)
  • Previous experience working with people with dementia

This position is subject to DBS checks under the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975.

Salary: £24,625 to £25,674 per annum, pro-rata.
