Job summary

Birmingham and Solihull Mental Health NHS Foundation Trust is seeking a Community Recovery Worker to join the Deaf Community Recovery Team for a 6 month fixed term. The role supports Deaf service users across inpatient and community settings within a large catchment area to promote mental health recovery and independence.

You will facilitate recovery-focused interventions, assist in discharge planning, and work across boundaries to promote social inclusion.

Responsibilities

• Provide individual support to Deaf service users in inpatient and community settings, in their own homes, to promote mental health recovery and independence.
• Support transition to discharge from inpatient wards and facilitate joint working.
• Promote independent living and social/leisure activities, focusing on service users' direct needs.
• Work across boundaries of care to promote social inclusion and maximise service effectiveness.
• Assist in delivering evidence-based practice, including advice and support for service users and carers within a multidisciplinary setting.
