• Take a person-centred approach using positive risk-taking to help individuals pursue goals.
• Build strong relationships with people you support, families, health professionals and social workers.
• Develop and review individual support plans, acting as a key link worker to reflect needs and aspirations.
• Support and guide colleagues to ensure safe, high-quality care and medication duties where needed.
