North East Ambition is seeking a Customer Advice and Support Worker to join their Adult Social Care and Health Integration team. This role involves supporting service users facing challenges related to Housing, Homelessness, and Domestic Abuse, while working in a collaborative, multi-disciplinary environment. This is an exciting time for Middlesbrough as we develop ACT (Accessing Change Together), which integrates council staff and partner organisations to provide a range of support and interventions for service users needing assistance with Housing Support, Homelessness, Domestic Abuse, and Substance Misuse. ACT is an integrated service, meaning multiple support services work together using a shared case load management system.
This integration aims to simplify access for service users to the support they need, ensuring they receive appropriate help, and to facilitate collaboration among council staff and partner organisations through shared information, ultimately improving outcomes. Recognising that service users may require help across several support areas, the teams adopt a multi-disciplinary approach, developing risk assessments and support plans tailored to individual needs. The team collaborates with other services, including Adult and Children’s Social Care and Public Health, to develop consistent pathways and effective processes.
We have multiple opportunities within ACT across three interconnected teams: Front Door, Housing Solutions, and Community Interventions.
We seek staff who are driven to contribute to the ACT vision, have experience working effectively across various services, and possess the skills, passion, and resilience to work creatively with service users requiring ongoing support related to Housing, Homelessness, and/or Domestic Abuse.
While in this role, there will be development opportunities, along with flexible working arrangements, including a blended environment of home and office working where appropriate.
