Job summary

Gloucestershire Health and Care NHS Foundation Trust is seeking a Band 5 nurse to join the Crisis Resolution and Home Treatment Team. The role centers on rapid assessment, short-term home treatment, telephone support and reducing hospital admissions.

You will share a caseload of 20–30 service users and work within a 24/7 service alongside a multidisciplinary team. Applicants should have an RMN qualification or an equivalent, with registration by the NMC/HCPC.

Qualifications

• RMN, Social Work or Occupational Therapy qualification, or awaiting results.
• NMC and HCPC registrations.
• Commitment to continuing professional development.

Responsibilities

• Take part in comprehensive assessment, planning and short-term intervention for psychiatric crises.
• Provide face-to-face and telephone advice tailored to service users’ needs.
• Hold responsibility for a shared caseload of 20–30 service users.
• Contribute to rapid assessment and delivery of crisis resolution or home treatment.
• Work as part of a multidisciplinary MHARS team providing 24/7 service.
• Demonstrate professional accountability for service users’ care.
• Support community-based care and promote recovery and social inclusion.
• Assist with hospital discharge planning and liaison with partner agencies.
