Job summary

Qualifications

• Master's degree from an accredited institution; degree verified.
• Current state licensure as LCSW, LMFT, CMHC/LCPC, or Psychologist.
• Excellent written and verbal communication skills.
• Computer skills (email, word processing).

Responsibilities

• Diagnoses and treats mental, emotional, and behavioral disorders; modify care plans as patient status changes.
• Refer patients to services, programs, groups, or other providers to support well-being.
• Collaborate with the multidisciplinary team to develop discharge plans for continuity of care.
• Organize and prioritize daily work by assessing patient needs across responsibilities.
• Conduct psychosocial assessments and provide crisis, bereavement, and behavioral health interventions as needed.
• Educate and coordinate with care teams and outside agencies to ensure timely, quality care and discharge.
• Provide staff support/debriefing through Employee Assistance Program when needed.
