South West London and St George's Mental Health NHS Trust is recruiting a dedicated Community Psychiatric Nurse to join the Richmond Home Treatment Team. based in Livingston House, Teddington, the role provides crisis assessments and short-term home-based care as an alternative to admission.

You will work within a multidisciplinary team, develop recovery-focused care plans with service users and carers, and support early discharge from wards.

Qualifications

• Current professional registration in relevant clinical subject/practice (e.g., Registered Mental Health Nurse)
• Experience in community or outpatient settings with crisis assessment
• Ability to work in a multidisciplinary team and maintain clinical documentation

Responsibilities

• Undertake comprehensive mental health, psychosocial and risk assessments in crisis situations
• Formulate and implement collaborative, recovery-focused care plans with service users and carers
• Provide intensive home treatment as an alternative to hospital admission and support early discharge
