Job summary

Cornwall Partnership NHS Foundation Trust is seeking an enthusiastic Mental Health Nurse to join our Community Mental Health Team. You will assess care needs, plan and deliver recovery-focused care across local communities, ensuring high-quality support for service users.

The role requires knowledge of the CPA process and Mental Health Act, with flexibility to travel across seaside towns and rural locations. Support and supervision are provided in a permanent Band 5 post.

Qualifications

• Registered Mental Health Nurse with appropriate qualification and registration.
• Experience in mental health settings and independent practice.
• Knowledge of CPA process and Mental Health Act.

Responsibilities

• Assess care needs and clinically manage a defined caseload within the Trust’s Mental Health Service.
• Develop, implement and evaluate programmes of care using care coordination.
• Interface between primary and secondary care to provide focused, coordinated packages of care.
