Adult North Team - Adult Social Worker (Adult SW) – Contract Opportunity

About the Role

As an Adult North Team - Adult SW, you will work within a small, dynamic team based in North. You will conduct Care Act assessments, reviews, CHC checklists, Discharge to Assess (DTA), and Care of the Person (COP) work as needed. The role involves participating in a duty rota to ensure client needs are met efficiently and compassionately.

Qualifications

• Proven experience as a Social Worker, preferably within adult social care
• HCPC registration is mandatory
• Experience in completing assessments, reviews, and care planning
• Ability to work effectively as part of a team and independently
• Flexibility to work in a hybrid setting (minimum 2 days in the office)
• Comprehensive understanding of the Care Act and related legislation
