We are looking for a Consultant ACP to join our Trust supporting sites across East and West Kent.
The Consultant Practitioner for Community Hospitals co-leads the medical provision for Community Hospitals, alongside the Consultant Clinical Lead.

You will be a clinical expert in Frailty and will fulfill the 4 domains of Advanced Clinical Practice:
