Job summary

Alternative Futures Group is seeking a care professional to lead the mobilisation of new complex care packages across the North West. You'll use clinical knowledge to shape personalised care plans and work with individuals, families and healthcare peers to deliver safe, independent living at home.

Through hands-on training and shadowing, you’ll help establish support packages and support new teams to develop skills and confidence from day one.

Responsibilities

• Lead mobilisation of new complex care packages across the North West.
• Use and build clinical & specialist knowledge through hands-on experience and specialist training in areas such as medication management, respiratory care, suction, enteral feeding, seizure management and more.
• Work closely with the individual, their family and wider healthcare professionals to understand needs and build support around them.
• Shadow-train and support new care teams to develop skills, knowledge and confidence to provide high-quality care.
• Work across different packages and locations to broaden complex care experience.
• Make a real difference from day one by helping establish a home-based support package for independent living.
